Overview

Certified Nursing Assistant CNA, Skilled Nursing Unit LTC, Mercyhealth Harvard Care Center

- 40 hours / 2 weeks

- Evening Shift (2:00P - 10:30P)

- Weekend and Holiday Rotation

Responsibilities

  • Provides resident care as assigned and per individual resident routine, according to standards of care, under CMS Federal and State Regulations for Skilled Nursing Facilities, and facility policies and procedures.
  • The ability to identify and document in the medical record the resident's usual and current cognitive patterns, mood and behavior, and whether these present a risk to the resident or others, identifying and documenting how the resident typically displays personal distress such as anxiety or fatigue, recognizing a change in the resident and the resident's functioning and the ability to describe and report such changes to the nurse such as negative verbalizations and vocalizations, facial expressions of grimacing, frowning, or fright, changes in gait, skin color, vital signs, behaviors such as resisting care, distressed pacing, irritability, depressed mood, and or decreased participation in usual physical and/or social activities, loss of function or inability to perform Activities of Daily Living, difficulty eating or loss of appetite and difficulty sleeping according to CMS RAI requirements.
  • Performs and documents daily range of motion exercises, assisting the resident to transfer from bed to wheelchair, assisting residents daily with bathing, dressing, grooming, transfers and ambulating through the facility, toileting, and eating.
  • Assists residents to attend activities of their choice
  • Grooming residents as they wish to be groomed (e.g., removal of facial hair for women, maintaining the resident's personal preferences regarding hair length/style, facial hair for men, and clothing style.)
  • The ability to describe the care, services and expected outcomes of care they provide; have knowledge of the care and services provided by therapists; have an understanding of the expected outcomes of this care and understand the relationship to the care they provide.
  • The ability to identify and document , to the extent possible, factors that may underlie the resident's expressions of distress, as well as applying knowledge of lifelong patterns, preferences, and interests for daily activities to enhance quality of life and individualize routine care.
  • The ability to identifying and understanding that the resident with dementia may be responding predictably given the situation or surrounding. For example, being awakened in the middle of the night in his/her bedroom by staff and not recognizing the staff could elicit an aggressive response. Matching activities for a resident with dementia to his/her individual cognitive and other abilities and tracking the specific behaviors in that resident based on the assessment.
  • The ability to identifying and document how the resident communicates physical needs such as pain, discomfort, hunger or thirst, as well as emotional and psychological needs such as frustration or boredom; or a desire to do or express something that he/she cannot articulate;
  • Answers call lights in a timely and appropriate manner ensuring that all of the residents needs to be provided by C.N.A. are met.
  • Documents all daily care provided according to Federal and State Regulations and standards of care
  • Maintains a clean work and patient environment.
  • Maintains supplies, including scanning, stocking and ordering new supplies and disposing of expired or damaged supplies.
  • Floats to various units based on identified needs.
  • Participates in department Quality Assurance and Performance Improvement activities.
  • Provide leadership and training to new CNA's.

Education and Experience

  • Completion of a CNA program.

Certification/Licensure

  • Current CNA certificate in applicable state.
  • Approved CPR or BLS certification required within 90 days.

Special Physical Demands

The Special Physical Demands are considered Essential Job Function of the position with or without accommodations.
While performing the duties of this Job, the employee is regularly required to reach with hands and arms and talk or hear. The employee is frequently required to stand; walk and use hands to finger, handle, or feel. The employee is occasionally required to sit; stoop, kneel, crouch, or crawl, taste, and smell. The employee must frequently lift up to 10 pounds, regularly lift up to 25 pounds, and occasionally lift up to 50 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision and depth perception.
