Job summary

Transitions Healthcare Washington PA is seeking compassionate CNAs for full-time roles. A $6,000 sign-on bonus is available, paid over a year, with comprehensive benefits and on-demand pay options.

Ideal candidates hold PA CNA certification (or eligibility with PA work eligibility via Ohio/WV credentials). Prior long-term care or hospital experience is preferred but not required in a fast-paced, team-focused environment.

Qualifications

• CNA certification required or eligibility to work in PA with an Ohio or WV credential.
• Current CPR certification preferred.
• Experience in long-term care, skilled nursing, rehab, or hospital setting is preferred but not required.

Responsibilities

• Provide compassionate, resident-centered care including bathing, dressing, grooming, toileting, feeding, oral hygiene and mobility.
• Assist with repositioning, transfers, and ambulation ensuring comfort, dignity and independence.
• Obtain and document vital signs, weights, intake/output and other clinical observations.
• Collect routine specimens as directed and document results.
• Perform restorative nursing activities per care plans.
• Notice changes in residents and report to licensed staff promptly.
• Respond to call lights and resident requests in a timely manner.
• Assist with admissions, transfers, and discharges and maintain clean, safe rooms.

Certified Nursing Assistant (CNA) Responsibilities

• Provide compassionate, resident-centered care while assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and mobility.
• Assist residents with repositioning, transfers, and ambulation while promoting comfort, dignity, and independence.
• Obtain and accurately document vital signs, weights, intake and output, and other assigned clinical observations.
• Collect routine specimens as directed.
• Perform restorative nursing activities in accordance with resident care plans.
• Observe residents for changes in physical or emotional condition and promptly report concerns to the licensed nursing staff.
• Respond promptly to call lights and resident requests.
• Assist with admissions, transfers, and discharges.
• Maintain clean, organized, and safe resident rooms and common areas.
• Complete rounding assignments, including safety and comfort checks.
• Accurately document care provided in the electronic medical record.
• Communicate effectively with residents, families, visitors, and members of the interdisciplinary healthcare team.
• Follow all infection prevention, safety, and resident rights policies.

Qualifications

• Current Pennsylvania Certified Nursing Assistant (CNA) certification required, or eligibility to work in Pennsylvania with an Ohio or West Virginia credential.
• Current CPR certification preferred.
• Previous long-term care, skilled nursing, rehabilitation, or hospital experience is preferred but not required.
