Transitions Healthcare Washington PA is actively seeking a Certified Nursing Assistant (CNA) to join our caring team in a full-time position in Washington, PA. You will provide compassionate, resident-centered care across activities of daily living and assist with mobility, vital signs, and documentation.

We offer a $6,000 sign-on bonus (paid over a year) and a comprehensive benefits package. Candidates licensed in PA, OH, or WV are encouraged to apply, with relocation not required.

Qualifications

• Current Pennsylvania CNA certification or eligibility to work in Pennsylvania with an Ohio or West Virginia credential.
• Current CPR certification preferred.
• Previous long-term care, skilled nursing, rehabilitation, or hospital experience is preferred but not required.
• Compassionate, dependable, and committed to providing exceptional resident care.
• Strong communication, teamwork, and time management skills.
• Ability to work effectively in a fast-paced healthcare environment.

Responsibilities

• Provide compassionate, resident-centered care while assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and mobility.
• Assist residents with repositioning, transfers, and ambulation while promoting comfort, dignity, and independence.
• Obtain and accurately document vital signs, weights, intake and output, and other assigned clinical observations.
• Collect routine specimens as directed.
• Perform restorative nursing activities in accordance with resident care plans.
• Observe residents for changes in physical or emotional condition and promptly report concerns to the licensed nursing staff.
• Respond promptly to call lights and resident requests.
• Assist with admissions, transfers, and discharges.
• Maintain clean, organized, and safe resident rooms and common areas.
• Complete rounding assignments, including safety and comfort checks.
• Accurately document care provided in the electronic medical record.
• Communicate effectively with residents, families, visitors, and members of the interdisciplinary healthcare team.
• Follow all infection prevention, safety, and resident rights policies.
