Job summary

Ovation Hospice in Utah is seeking an outstanding Hospice Aide to join our compassionate team as a Hospice CNA. You will provide personal care under the supervision of a registered nurse, ensuring patients come first in every interaction.

The role requires at least 1 year of CNA/hospice/home health aide experience, fulfillment of Utah state training requirements, and a valid driver's license with insured vehicle. We offer a competitive hourly wage and full benefits.

Qualifications

• Meet training requirements by the state of Utah.
• One year of CNA, hospice, or home health aide experience.
• Be at least 18 years old.
• Valid driver’s license and insured vehicle.
• Good verbal and written communication; able to work in a team and independently.

Responsibilities

• Document care provided according to organization policy.
• Report deviations from the Plan of Care to the Case Manager.
• Use Durable Medical Equipment safely.
• Take vital signs when on Hospice Aide Plan of Care.
• Foster cooperative effort among personnel and participate in team conferences.
• Provide care and observation of patient’s skin to prevent tissue breakdown.
• Assist with ambulation, transfer, range of motion as documented.

Responsibilities

• Document care provided according to organization policy.
• Report any deviations from the Plan of Care to the Case Manager
• Use Durable Medical Equipment safely.
• Take vital signs when on Hospice Aide Plan of Care.
• Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care any by active participation in team and staff conferences.
• Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.
• Assist with ambulation, transfer, range of motion if documented on the Hospice Aide Plan of Care.
  

Qualifications

• At least 18 years old.
• Must be a licensed driver with an insured automobile in good working order.
