Description
Provides hospice patients with personal care and assistance as directed by the individualized Plan of Care for each assigned patient. Visit expectation is 20-25 visits per week. Works with the other members of the YoloCares Interdisciplinary Team to increase the level of comfort, and to support a safe and healthy environment for each assigned patient.

Description
Provides hospice patients with personal care and assistance as directed by the individualized Plan of Care for each assigned patient. Visit expectation is 20-25 visits per week. Works with the other members of the YoloCares Interdisciplinary Team to increase the level of comfort, and to support a safe and healthy environment for each assigned patient.
PROVISION OF PERSONAL CARE SERVICES
- Follows universal precautions in delivering all patient care.
- Assists with or provides personal care, e.g., shower, tub bath, bed bath, oral hygiene, shaving, shampooing, cleaning nails, assists to toilet, etc as desired by a patient, directed by a specific patient’s written Plan of Care, and supervised by a Registered Nurse.
- Changes patient clothing and bed linen as requested.
- Increases patient comfort and safety by prescribed rehabilitative measures such as positioning, ambulating and assistance with transfers. Uses basic adaptive equipment with debilitated patients when the Plan of Care so requires.
- Performs routine procedures such as feeding non-compromised patients; making occupied and unoccupied beds; cleaning patient equipment.
- Demonstrates and teaches care techniques to patient’s caregiver when appropriate.
- Records vital signs when requested by the Plan of Care and/or as requested by patient’s Registered Nurse.
- Assists a patient in taking self-administered oral medications as approved by the patient’s assigned Registered Nurse.
- Encourages simple measures for patient’s bowel and bladder regularity.
- Observes/monitors patient’s physical and emotional condition and reports changes to the patient’s Registered Nurse or the Clinical Supervisor in a timely manner.
- Assists patient/family/caregivers in coping with terminal illness and impending death.
- Manages assigned case load by planning weekly schedule for visit to assigned patients, keeps office and families clearly informed of schedule, and meets scheduled appointments with families whenever possible.
- It is expected that patient requests will be met whenever possible, that patients and families will be notified by phone if a visit time will be more than 30 minutes earlier or later than scheduled, and that an average patient care visit will last about an hour.
- Ensures that information about a patient and family is protected and that confidentiality is maintained at all times according to YoloCares polices and to the requirements of the Health Insurance Portability and Accountability Act (HIPAA).
PROVISION OF HOMEMAKER SERVICES
- Provides homemaker services of cleaning patient’s room, washing patient’s personal laundry, preparing and assisting with a patient’s meal, and other homemaking duties, when the patient’s Plan of Care so directs.
PROFESSIONAL DOCUMENTATION
- Ensures that professional Hospice Aide visit notes and other required records are written and turned in to the Administrative Assistant in Patient Care Services or designee, in compliance with state and federal regulations and with YoloCares policies and procedures.
- Assures that documentation of care is accurate legible, honest and clearly stated, and that medical terminology is used appropriately when reporting on a patient’s condition.
- Records care given to a patient, as well as pertinent observations and significant changes such as reactions to care, symptoms observed, mental and physical status changes in the patient, and coping changes noted in patient and/or caregiver(s).
INTERDISCIPLINARY TEAM (IDT) COLLABORATION/COMMUNICATION
- Participates in the weekly interdisciplinary team (IDT) meetings and actively participates in care planning and evaluation discussions related to assigned patients.
- Communicates the need to discuss significant new patient/family situations to the primary hospice Registered Nurse and/or the Clinical Supervisor promptly, so that a care planning team can convene in person or by phone.
- Recognizes the knowledge and skills of other disciplines represented on the IDT and advocates for coordinated IDT intervention(s) with the patient and family.
- Participates with colleagues/IDT members in identification and discussion of ethical conflicts with arise in caring for terminal patients and their families.
- Consistently meet all standards of conduct delineated in the YoloCares Integrity Program: Standards of Conduct: A Guide to Compliance and Ethics.
PROFESSIONAL & TECHNICAL KNOWLEDGE DEVELOPMENT
- Participates in and successfully completes the 12 CE training classes that YoloCares provides annually as required to maintain active certification as a CHHA by the California Department of Health Services.
- Arranges for any additional classes needed to maintain CHHA license as required by California Department of Health Services.
- Identifies personal education needs, seeks formal programs, participates in professional educational activities or seeks other avenues (i.e. reading articles/books etc.) to meet these needs. Provides Clinical Supervisor with feedback concerning timely topics for Hospice Aide education and discussion within the YoloCares program.
- Maintains a working style which demonstrates teamwork, constructive time management, effective conflict resolution and problem solving with patient, families and all other members of the IDT.
- Participates in the evaluation(s) of own performance by identifying strengths and weaknesses and by developing plans to capitalize on strengths and improve areas of weakness.

Requirements
Job Qualifications:
- Valid and active Certified Nursing Assistant (CNA) license and Home Health Aide (HHA) certification is required.
- Must be CPR Certified.
- Minimum 1-year Hospice experience preferred.
- Ability to make clinical observations and clearly communicate verbally and in writing.
- Strong interpersonal skills and ability to quickly build rapport with others.
- Ability to successfully meet agency background screening requirements.'Ability to safely lift at least 50lbs

Pay Range: $52,000 - $56,160 / Yearly
