Responsibilities

1. Identify and report resident problems and condition changes to Charge Nurse.
2. Bath, dress, groom, and assist residents with daily activities.
3. Record food and fluid intake accurately.
4. Assist with feeding, monitor trays, answer call lights, and deliver/remove trays.
5. Reposition residents, change linens, and perform baths.
6. Assist with resident transfers, toileting, and hygiene care.
7. Monitor and report vital signs and resident condition.
8. Follow safety, infection control, and emergency procedures.
9. Participate in ongoing training and maintain professional appearance.
10. Document care activities and incidents accurately.
11. Collaborate with healthcare team to ensure quality resident care.

Qualifications
