Avamere Rehabilitation at Ridgemont in Port Orchard, WA is seeking a Certified Nursing Assistant (CNA) to provide routine daily nursing care in accordance with resident care plans. This full-time position also offers PRN shifts and flexible scheduling (Days or Eves).

The role involves assisting residents with personal care, documenting changes, and collaborating with nurses to support residents’ quality of life. A CNA license and CPR/BLS are required.

Qualifications

• Have a 10th grade education or above.
• Be a licensed certified nursing assistant in this state.
• Must maintain and have an active CPR/BLS during employment.

Responsibilities

• Provide routine daily nursing care and services per resident care plans.
• Check residents to ensure personal care needs are met per their wishes.
• Review care plans daily and note changes in residents’ daily care.
• Inform the nurse supervisor of changes to ensure updates on the care plan.
• Assist residents with daily needs and escort to activities and appointments.
• Create a warm, patient, and cheerful environment for residents.

Job Summary

The primary responsibility as a Certified Nursing Assistant is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan.

Essential Duties And Job Responsibilities

• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes
• Review resident care plans daily to determine if changes in the resident's daily care
• routine have been made on the care plan
• Inform the nurse supervisor of any changes in the resident's condition so that
• appropriate information can be entered on the resident's care plan
• Assist residents with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments
• Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment
• Ensure residents' rooms are ready for receiving and help residents feel comfortable
• Participate in facility surveys by authorized government agencies
• Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator
