Job summary

Autumn Hospice is hiring a Certified Nursing Assistant (CNA) to join the South Team. Responsibilities include assisting with ADLs, transfers, ROM, feeding, and vital signs, while ensuring compassionate, patient-centered end-of-life care.

Travel across the assigned territory is required, with a valid driver’s license and reliable transportation necessary. Education requires a High School Diploma or equivalent and current CNA certification.

Qualifications

• Must have a current state Certified Nursing Assistant (CNA) certification.
• Previous hospice or home health experience preferred.
• No prior hospice experience required; one year of CNA experience preferred.
• Must be willing and able to travel throughout the assigned territory.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Provides or assists with ADLs and personal hygiene.
• Assists with toileting, positioning, and transfers, including bed-bound patients.
• Assists with prescribed exercises and ROM as instructed by Hospice Nurse.
• Assists with light housekeeping in patient’s living area.
• Feeds and assists patients with meals as appropriate.
• Obtains vital signs under supervision of the Hospice Nurse.
• Reports changes in patient condition to Hospice Nurse and facility nurse when applicable.
• Assists or reminds patients to take medications per regulations and accrediting bodies.
• Maintains patient confidentiality and HIPAA compliance.

The Certified Nursing Assistant (CNA) will be responsible for, but not limited to:

Responsibilities

• Provides or assists with activities of daily living (ADLs), including personal hygiene, grooming, mouth care, skin care, and nail care.
• Provides assistance with toileting, positioning, and transfers, including frequent turning and positioning of bed-bound patients.
• Assists with prescribed exercises and range-of-motion (ROM) activities that the patient and Hospice Aide have been instructed on by the Hospice Nurse.
• Assists with light housekeeping of the patient's personal living area or room, including changing bed linens and patient laundry as needed.
• Feeds and assists patients with meals as appropriate.
• Obtains temperature, pulse, and respirations as directed and supervised by the Hospice Nurse.
• Reports any changes in the patient's condition to the Hospice Nurse and, when the patient resides in a facility, to the appropriate facility nurse. Documents the information as required, including the date and time of the report and the individual receiving the information.
• Assists or reminds patients to take medications in compliance with applicable regulations and the requirements of appropriate regulatory and accrediting bodies.
• Provides compassionate and respectful care to hospice patients and their families while maintaining patient dignity, comfort, and quality of life throughout end-of-life care.
• Communicates effectively with the Hospice Nurse and other members of the interdisciplinary care team regarding patient needs and changes in condition.
• Maintains patient confidentiality and complies with all applicable hospice policies, HIPAA requirements, and regulatory standards.

Education

• High School Diploma or equivalent.
