Job summary

Waukegan Health and Rehab is seeking a Certified Nursing Assistant (CNA) to provide routine daily nursing care to residents in accordance with care plans and supervisor directions.

You will participate in activities of daily living, mobility assistance, grooming, dining support, and infection control, while documenting care and communicating changes to the nursing team. On-site with a supportive care staff.

Qualifications

• Must have current and active nursing aide certification in Wisconsin.

Responsibilities

• Resident centered care including assistance with activities of daily living such as positioning, moving and restorative care.
• Mobility, toileting, and bathing.
• Grooming: oral, shaving and nail care.
• Dining assistance.
• Dressing and undressing.
• Infection control and prevention.
• Nutrition and elimination.
• Comfort care and end of life care.
• Answer call lights timely and promptly.
• Report all changes in the resident's condition to your assigned nurse as soon as possible.
• Report all accidents and incidents you observe on the shift when they occur.
• Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
• Be a team player working in coordination with all disciplines in the facility.
