Sapphire Health Services, LLC in Tigard, Oregon is seeking a dedicated Certified Nursing Assistant (CNA) to provide direct care to residents under licensed nurses. You will assist with activities of daily living, promote independence, and contribute to a warm, respectful environment.

Responsibilities include bathing, grooming, mobility assistance, vital sign monitoring, responding to call lights, participating in activities, maintaining infection control, and collaborating with the care team.

Formación

• Must be 18 years or older.
• Active CNA license in the state of practice.
• Completion of a state-approved CNA training program.
• Compassionate, patient-centered caregiver.
• Strong communication and interpersonal skills.
• Reliable with a strong work ethic.
• Ability to assist with mobility and lifting.
• Ability to stand, walk, and reach for extended periods.
• Background check clearance required.

Responsabilidades

• Assist residents with mobility and transfers using proper body mechanics.
• Monitor vital signs, intake/output, and report changes to licensed staff.
• Respond to call lights promptly with compassion and empathy.
• Support therapeutic and recreational activities to enhance residents’ well-being.
• Maintain a clean, safe environment with infection-control practices.
• Collaborate with interdisciplinary team to tailor care to residents’ needs.
• Provide emotional support and companionship to residents.
• Perform other duties as assigned.

JOB SUMMARY:

As a Certified Nursing Assistant (CNA), you will play a crucial role in providing direct care and support to our residents under the supervision of licensed nursing staff. You will assist residents with activities of daily living, promote their independence and well-being, and contribute to a warm and welcoming environment where residents feel valued and respected.

RESPONSIBILITIES:

• Adheres to the policies and procedures of the facility.
• Provide direct care to residents, including bathing, grooming, dressing, toileting, and feeding, according to individual care plans and preferences.
• Assist residents with mobility and transfers, using proper body mechanics and assistive devices as needed.
• Monitor and record vital signs, intake and output, and other relevant information, reporting any changes or concerns to licensed nursing staff.
• Respond promptly to residents call lights and requests for assistance, demonstrating compassion, patience, and empathy in all interactions.
• A CNA will assist with the implementation of therapeutic and recreational activities designed to enhance residents' physical, cognitive, and emotional well-being.
• Maintain a clean, safe, and organized environment for residents, ensuring compliance with infection control protocols and safety regulations.
• Collaborate with interdisciplinary team members to promote continuity of care and meet the individual needs and preferences of residents.
• Provide emotional support and companionship to residents, fostering meaningful relationships and promoting socialization and engagement.
• Other duties as assigned.

REQUIREMENTS:

• Must be 18 years of age.
• Active Certified Nursing Assistant (CNA) License in the State of practice.
• Successful completion of a state-approved Certified Nursing Assistant (CNA) training program.
• Compassionate and patient-centered approach to resident care.
• Strong communication and interpersonal skills.
• Reliable and dependable with a strong work ethic.
• Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.
• Must be able to stand, walk, and reach for extended periods.
• Ability to pass a background check.
