Job summary

Journey is seeking a dedicated Care Team member in Savannah, GA to provide compassionate patient care and support. You will assist with activities of daily living and emotional support, while maintaining safety and dignity for residents.

Responsibilities include monitoring vitals, documenting changes, and ensuring infection control in collaboration with the healthcare team. Certification in CNA/SRNA/GNA is required, with ongoing CE hours to maintain licensure.

Qualifications

• Certified in the state as CNA/SRNA/GNA.
• Must attend a minimum of 12 hours continuing education programs provided by the center to maintain certification.

Responsibilities

• Provide activities of daily living assistance and companionship.
• Monitor vital signs and report changes to the nursing staff.
• Maintain infection control and collaborate with the care team to ensure high-quality care.

Major Duties and Responsibilities

• Health Monitoring and Reporting: Monitor vital signs, document patient information accurately, and report changes or concerns to the nursing staff.
• Environment and Team Collaboration: Follow infection control protocols, maintain a clean and safe environment, and collaborate with the healthcare team to ensure high-quality care.
