Qualifications:
  • Certified Nursing Assistant (CNA) license (required)
  • 12 months caregiving experience in settings like home care, assisted living, skilled nursing, hospital, or hospice (required)
  • Current CPR certification (preferred)
  • Hospice experience (preferred)
  • Excellent customer service and communication skills
  • Problem-solving and time management skills
  • Ability to work with electronic medical records
  • Ability to lift 50 lbs. and understand body mechanics
  • Valid driver's license (required)
Responsibilities:
  • Assist with daily personal care activities
  • Review and follow patient care plans
  • Help with lifting, transferring, and mobility
  • Support with nutrition and hydration
  • Maintain electronic medical records
  • Work under the supervision of a Case Manager RN
