Job description

Summary

The Home Health CNA is responsible for providing patients with in-home personal care and designated health-related services to maintain the patient's physical and emotional well-being, following the written plan of care, Medicare/Medicaid regulations, and agency policies and procedures.

Pay range: $25 per visit, depending on experience.

Responsibilities
  • Reports any unusual circumstances in the patient's condition or home environment to the primary RN/Patient Care Manager.
  • Follows the current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs), including mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting.
  • Performs incidental household services essential to the patient's health care at home as assigned, necessary to prevent or postpone institutionalization.
  • Completes a clinical note for each visit within required timeframes, which must be incorporated into the patient's record.
  • Participates in the in-services program, completing at least six hours of in-services every six months.
  • Participates in daily reports and supports the care management model within the agency.
  • Participates in staff conferences regarding patient care.
  • Participates in QA/QI plans or processes.
  • Provides care according to the aide plan of care/assignment sheet and in accordance with State Practice Act or regulations.
  • Records and notifies the RN/Patient Care Manager of any variations from established parameters or changes in the patient's condition.
Education and Experience

License Requirements:

  • Successful completion of a competency evaluation.
  • Current CPR certification required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Additional State Specific Requirements:

  • CT: Must have completed a training program approved by the state of Connecticut of at least 75 hours.
Skill Requirements
  • Must be mature and able to effectively handle the demands of a home health aide position. Able to read, write, and carry out directions promptly and accurately.
