Qualifications

• Current CNA certification.

Responsibilities

• Follows the client's plan of care as assigned.
• Delivers personal care services to clients as assigned.
• Assist with mobility and safe transfers.
• Monitor client vitals and health status.
• Help with light housekeeping and meal preparation.
• Administer medications as prescribed and provide reminders.
• Provide companionship and engage clients in activities.
• Report changes to the healthcare team or family.
• Maintain records and comply with HIPAA.

• Maintains confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
• Communicates information effectively to appropriate personnel.
