Job summary

Plaza Community Living Center is seeking a skilled Certified Nursing Assistant (CNA) to provide compassionate direct care to residents. Two full-time shifts are available: day 7am–3pm and night 11pm–7am. Join a team focused on compassionate, personalized care and opportunities for growth.

Responsibilities

• Conduct regular direct care rounds to monitor activity and assist with activities of daily living
• Maintain positive relationships with residents and patients
• Notify Charge Nurse of any resident changes in condition
• Obtain vital signs, weights, and heights as instructed
• Complete all assigned duties and documentation
• Coordinate for admissions, transfers, and discharges in Transitional Care Unit
• Maintain a safe, clean work area and follow infection control practices
• Perform other duties as assigned

Shifts available:

Day shift-7am to 3pm (Full-time schedule)

Night shift-11pm to 7am (Full-time schedule)

Certified Nursing Assistant ( CNA ) duties include but are not limited to:

• Conduct regular direct care rounds on assigned unit to monitor activity and ensure residents needs are met including assisting with activities of daily living
• Maintain positive relationships with the residents/ Patients
• Notify Charge Nurse of any resident/ patient change in condition
• Obtain vital signs, weights, and heights as instructed by charge nurse
• Complete all assigned duties and documentation as required
• Coordinate and prepare for new admissions, transfers, discharge of residents in the Transitional Care Unit
• Maintain safe and clean work area
• Ensure infection prevention and control practices
• Other duties as assigned
