Job summary

Sunplex Subacute Center is seeking a compassionate Certified Nursing Assistant (CNA) for the evening shift (2pm to 10pm). You will provide direct resident care, monitor residents’ conditions, and document accurately under the direction of the nursing team.

Qualifications

• CNA certification in good standing is required.
• Direct care and patient handling skills are needed.
• Ability to monitor and document residents' conditions as directed.

Responsibilities

• Conduct regular direct care rounds on assigned unit to monitor activity and ensure residents' needs are met.
• Maintain positive relationships with residents and patients.
• Notify Charge Nurse of any change in condition.
• Obtain vital signs, weights, and heights as instructed by charge nurse.
• Complete all assigned duties and documentation.
• Coordinate admissions, transfers, and discharges in the Transitional Care Unit.
• Maintain a safe, clean work area and adhere to infection prevention practices.

Full time | Evening: 2pm to 10pm

Certified Nursing Assistant ( CNA ) duties include but are not limited to:

• Conduct regular direct care rounds on assigned unit to monitor activity and ensure residents needs are met including assisting with activities of daily living
• Maintain positive relationships with the residents/ Patients
• Notify Charge Nurse of any resident/ patient change in condition
• Obtain vital signs, weights, and heights as instructed by charge nurse
• Complete all assigned duties and documentation as required
• Coordinate and prepare for new admissions, transfers, discharge of residents in the Transitional Care Unit
• Maintain safe and clean work area
• Ensure infection prevention and control practices
• Other duties as assigned
