Responsibilities

• Provide direct patient care to residents, including assistance with daily living activities such as bathing, dressing, and grooming
• Monitor and report changes in resident condition, including vital signs and other health metrics
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Support residents with activities of daily living (ADLs) and assist with mobility and transfer techniques
• Maintain accurate records and documentation of resident care and services
• Promote a safe and healthy environment for residents and colleagues

Requirements

• Current certification as a Certified Nursing Assistant (CNA) in the state of Pennsylvania
• High school diploma or equivalent required; some college or vocational training preferred
• Ability to work effectively in a fast-paced environment with varying levels of patient acuity
• Strong communication and teamwork skills
• Ability to lift, transfer, and move patients as needed
