Job summary

Qualifications

• CPR/First Aid certification preferred or required by the position.

• Reliable attendance and punctuality.
• Ability to pass required background checks and meet applicable employment requirements.
• Reliable transportation for traveling to client homes may be required.

Responsibilities

• Assist clients with bathing, grooming, dressing, toileting, and personal hygiene.
• Assist with mobility, transfers, positioning, and ambulation as appropriate.
• Monitor and report changes in a client's condition to the supervising nurse or care coordinator.
• Assist with meals and encourage proper nutrition and hydration according to the care plan.
• Provide companionship and emotional support.
• Assist with light housekeeping related to the client's care and safety.
• Take and document vital signs as directed and within the scope of training and authorization.
• Provide assistance with activities of daily living according to the client's care plan.
• Maintain accurate documentation of care provided.
• Follow individualized care plans and company policies and procedures.
• Communicate professionally with clients, family members, caregivers, and healthcare professionals.
• Maintain client privacy and confidentiality.
• Report safety concerns, incidents, or suspected abuse or neglect according to applicable requirements and company policy.
