Job summary

Solera at West Houston is seeking a compassionate Certified Nursing Assistant (CNA) to provide high-quality patient care in a skilled nursing setting. You will assist with ADLs, monitor patient condition, document care, and support safe, dignified daily living.

Qualifications

• Active CNA certification required.
• Ability to lift, move and transfer patients safely.

Responsibilities

• Deliver direct patient care including bathing, dressing, grooming, toileting, and feeding.
• Assist with patient transfers, ambulation, and repositioning using safe lifting techniques.
• Monitor, observe, and report changes in patient condition to the Charge Nurse.
• Document patient care and daily activities accurately.
• Follow individualized care plans to improve patient outcomes.
• Maintain a clean, infection-controlled environment and ensure equipment is functioning.
• Participate in shift handoffs and team communication.
• Follow all facility policies and procedures to promote dignity and safety.
