Job summary

Trilogy Health Services, LLC is seeking a Certified Nursing Assistant (CNA) in Fremont, OH to deliver daily nursing care according to each resident’s plan. You will provide companionship, monitor residents, and support the care team to enhance comfort and wellbeing at Valley View Healthcare Center.

Applicants should hold a state CNA certification and have 0–1 year of experience. The position is on-site, with a focus on compassionate, person-centered care and a safe, supportive work environment.

Qualifications

• High School Diploma or GED/HSE preferred.
• Active state CNA Certification required.
• 0–1 years of relevant experience preferred.

Responsibilities

• Assist residents with activities of daily living (hygiene, turning, lifting, positioning, and transportation).
• Observe and report changes in residents' condition and document findings.
• Collaborate with the care team to ensure resident comfort and well-being.
• Provide companionship, compassion, and encouragement to residents.
• Maintain a safe and comfortable environment for residents.

Position Overview

Certified Nursing Assistants (CNA) work alongside fellow caregivers to provide each of your residents with daily nursing care and services based on the resident’s plan of care and individual needs. CNAs also provide companionship, compassion, and encouragement to our residents.

Key Responsibilities

• Help residents with personal hygiene activities, turning, lifting, positioning, and transporting them when needed (activities of daily living)
• Observe and report changes in residents' condition and make independent decisions when needed
• Collaborate with the team to ensure patient comfort and well-being
• Provide companionship, compassion and encouragement to our residents
• Maintain a safe and comfortable environment
