Job summary

As a Hospice Aide (CNA) with Tapestry Hospice & Palliative Care, you’ll provide compassionate personal care to patients in their homes across Murray, Gordon, and Gilmer counties. You’ll follow care plans, observe conditions, and communicate changes to the RN Case Manager.

Qualifications

• Must live in Murray, Gordon, or Gilmer County.
• Current Georgia CNA Certification.
• Current CPR & First Aid Certification.
• Valid Driver's License, Reliable Vehicle & Auto Insurance.
• Ability to travel throughout assigned territory.
• Hospice, home health, or long-term care experience preferred.

Responsibilities

Position Summary

As a Hospice Aide (CNA), you are one of the most important members of our care team. You serve as the eyes, ears, and heart of patient care by providing personal care services, observing patient condition, and communicating changes to the Registered Nurse Case Manager.

You will follow individualized care plans, help patients maintain dignity and comfort, and provide support to families while working closely with the hospice interdisciplinary team.
