Job summary
Signature HealthCARE is seeking a Certified Nursing Assistant for our Carrollton Rehab & Wellness Center. You will provide direct resident care under licensed supervision, assist with ADLs, and support a compassionate, resident-centered environment.
The CNA role includes night shifts (10PM–6AM) with a strong emphasis on teamwork, safety, and quality of life for residents in a 78-bed facility. Join us to grow in a family-based organization.
Qualifications
- Current, active Nursing Assistant certification from state in which the facility operates.
- Effective verbal and written communication skills.
- Ability to work collaboratively within a nursing team and under licensed staff.
Responsibilities
- Provide ADLs within scope of practice as defined by state regulation and care plan.
- Record resident vital signs and other data as assigned.
- Ambulate, transfer residents with proper body mechanics.
- Turn and position residents to maintain comfort and skin integrity.
- Support resident rights and choices within the care environment.
- Promptly answer call lights and assist residents' needs.
- Assist in orientation and training of staff as requested.
- Perform other duties and special projects as assigned.

Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents in accordance with facility policies and procedures.
- Provide activities of daily living (ADLs) within the scope of practice as defined by state regulation and the resident’s plan of care.
- Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis.
- Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics.
- Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
- Routinely turn and position residents, as necessary.
- Recognize and support resident rights and choices within the care environment.
- Maintain established housekeeping standards within assigned duty areas.
- Promptly answer call lights and assist with resident needs.
- Assist in the orientation and training of other staff, as requested.
- Other special projects and duties, as assigned.
- Current, active Nursing Assistant certification from state in which the facility operates.
- Effective verbal and written communication skills.
- Ability to work collaboratively within a nursing team and under the direction of licensed nursing staff.
