Job summary

Continuum Care Hospice in Barnstable, MA is seeking a compassionate Full-Time Hospice Aide (CNA) to provide hands-on care under the nursing team, ensuring comfort and dignity for patients at the end of life.

Responsibilities include personal care, accurate documentation, and supporting families. Schedule is Monday–Friday, 8:00 am–4:30 pm, with a $5,000 sign-on bonus and comprehensive benefits.

Qualifications

• High School Diploma or GED required.
• Holds current unencumbered CNA certificate in MA.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• Must maintain CPR certification if state-mandated.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide personalized care following the plan of care and respond promptly to patient and family requests.
• Assist with personal care including bathing, oral hygiene, changing bedding, and grooming.
• Maintain exceptional customer service and patient satisfaction.
• Support families by sharing knowledge and guiding care for terminally ill loved ones.
• Maintain accurate documentation in the home and agency records.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$24.00 - $26.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
