Job summary

Continuum Hospice is seeking a compassionate Full-Time Hospice Aide (CNA) to work with our nursing team in Barnstable, providing hands-on care that supports patients at the end of life. You will follow care plans and help with daily activities, delivering comfort, dignity, and personal attention.

Candidates should have a high school diploma or GED, an active CNA certificate, CPR if required, and at least one year of CNA experience in acute or long-term care; hospice experience is a plus.

Qualifications

• High School Diploma or GED required.
• Holds current unencumbered CNA certificate in the state.
• Minimum one (1) year experience as a CNA in an acute care or long term care setting; hospice experience is a plus.
• Must possess and maintain CPR certification if state-mandated.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Provide personalized care following the plan of care to ensure patient comfort and well-being.
• Assist with personal care needs including bathing, oral hygiene, changing bedding, clothing, and shampooing.
• Maintain exceptional customer service and deliver high-quality care.
• Provide support and guidance to families caring for terminally ill loved ones.
• Maintain accurate documentation in patient home records and agency systems.

Continuum Hospice is seeking a Full Time Hospice Aide (CNA) to join our compassionate team, providing hands‑on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Monday - Friday, 8 am - 4:30 pm

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.

• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!

$24.00 - $26.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
