Job description

JOBSUMMARY:

Theprimary purpose of the Certified Nursing Assistant position is to provide each assignedresident with excellent daily nursing care and services in accordance with theresident’s assessment and plan of care, and as may be directed by yoursupervisor.

JOBDUTIES & RESPONSIBILITIES:

AdministrativeFunctions
  • Identify residents beforeadministering treatments, serving meals, etc. as necessary.
  • Record all entries on flowsheets, notes, charts, etc. in an informative and descriptive manner.
  • Use only authorizedabbreviations established by this facility when recording information.
  • Report all changes in theresident’s condition to the RN Team Leader as soon as possible.
  • Report and providestatement for all events, accidents and incidents you observe on the shiftas they occur.
Admission,Transfer, and Discharge Functions
  • Ensure that the resident’sroom is ready for receiving the resident (i.e., bed made, name tags up,admission kit available, etc.)
  • Greet residents and escortthem to their room.
  • Introduce resident tohis/her roommate, if any, and other residents and personnel asappropriate.
  • Make resident comfortable(i.e., put to bed, get water, etc.)
  • Inventory and mark theresident’s personal possessions as instructed.
  • Store resident’s clothing.
  • Assist residents withpacking their personal belongings when they are being transferred ordischarged.
  • Transport residents to newrooms or to the receiving area.
  • Assist withloading/unloading residents from vehicles as necessary.
PersonnelFunctions
  • Perform all assigned tasksin accordance with our established policies and procedures, and asinstructed by your supervisor.
  • Follow work assignments,and/or work schedules in completing and performing your assigned tasks.
  • Cooperate withinter-departmental personnel, as well as other facility personnel toensure that nursing services can be adequately maintained to meet theneeds of the residents.
  • Create and maintain anatmosphere of warmth, personal interest and positive emphasis, as well asa calm environment throughout the unit and the shift.
  • Meet with your shift’snursing personnel, on a regularly scheduled basis, to assist inidentifying and correcting problem areas, and/or the improvement ofservices and participate in Team Conference.
  • Report all complaints andgrievances made by the resident.
  • Notify the facility whenyou will be late or absent from work.
  • Report occupationalexposures to blood, body fluids, infectious materials, and hazardouschemicals to your supervisor.
PersonalNursing Care Functions
  • Participate in and receivethe nursing report upon reporting for duty.
  • Follow establishedpolicies concerning exposure to blood/body fluids.
  • Assist residents withhygiene needs to include:

·Dailydental and mouth care (i.e. brushing teeth/dentures, oral hygiene, specialmouth care, etc.)

·Bathing(i.e. bed bath, tub or shower bath, etc.) as directed

·Dressing/undressingas necessary

·Haircare needs (i.e. combing, brushing, shampooing, etc.)

·Nailcare (i.e. clipping, trimming and cleaning the finger/toe nails)

·Shavingmale residents

·Controllinghair growth on female residents (i.e. facial hair, under arms, on legs, etc.)as instructed

·Keepingresidents dry (i.e. change gown, clothing, linen, etc. when they become wet orsoiled)

  • Change bed linens. Keeplinens tight to avoid wrinkles from forming under the resident.
  • Make beds (occupied andunoccupied.)
  • Put extra linens on bed asrequested.
  • Position bedfast residentsin correct and comfortable position, utilizing positioning devices asindicated.
  • Assist resident with boweland bladder functions (i.e., take to bathroom, offer bedpan/urinal,portable commode, etc.)
  • Maintain intake and outputrecords as instructed.
  • Keep incontinent residentsclean and dry.
  • Check and report bowelmovements and character of stools as instructed.
  • Assist residents inpreparing for medical tests and appointments (i.e., lab work, x-ray,therapy, dental, physician dialysis, etc.)
  • Assist residents inpreparing for activity and social programs (i.e. church, parties,visitors, physician, dialysis).
  • Assist in transportingresidents to/from appointments, activity and social programs, etc. asnecessary.
  • Assist with lifting,turning, moving, positioning, and transporting residents into and out ofbeds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Assist residents to walkwith or without self-help devices as instructed.
  • Provide eye and ear care(i.e., warm/cold compresses, removing wax and hair, cleaningeyeglasses/hearing aids, dentures, etc.) as instructed.
  • Assist in preparing theresident for a physical examination.
  • Measure and recordtemperatures, pulse, and respirations, as instructed.
  • Answer resident callspromptly.
  • Check each residentroutinely to ensure that his/her personal care needs are being met inaccordance with his/her wishes.
  • Assist with the care ofthe dying resident as directed.
  • Provide post-mortem careas instructed.
SpecialNursing Care Functions
  • Observe and report thepresence of red areas and skin breakdowns to prevent decubitus ulcers.
  • Report injuries of anunknown source, including skin tears.
  • Provide daily indwellingcatheter care.
  • Provide daily perinealcare.
  • Perform non-sterile(clean) vaginal irrigation/douches.
  • Assist with theapplication of slings, elastic bandages, binders, etc.
  • Give tepid sponge baths.
  • Provide daily range ofmotion exercises and record data as instructed.
  • Turn bedfast residents atleast every two (2) hours.
  • Perform diabetic urinetesting, as directed.
  • Observe disoriented andcomatose residents. Record and report data as instructed.
  • Turn all medications foundin the resident’s room over to the Medication Administration Manager.
  • Watch for and report anychange in room temperature, ventilation, lighting, etc. to the RN TeamLeader.
FoodService Functions
  • Prepare residents formeals (i.e., take to bathroom, wash hands, comb hair, raise bed, positiontables, place clothing protectors, take to/from dining room, etc.)
  • Serve food trays. Assistwith feeding as indicated (i.e., cutting foods, feeding, assist in diningroom supervision, etc.)
  • Assist residents withidentifying food arrangements (i.e. informing resident with sight problemof foods that are on his/her tray, where it is located, if it is hot/cold,etc.)
  • Record the resident’s foodintake. Report changes in the resident’s eating habits.
  • Keep residents’ waterpitchers clean and filled with fresh water and within easy reach of theresident.
  • Serve between meal andbedtime snacks.
  • Perform after meal care(i.e. remove trays, clean resident’s hands, face, clothing, take tobathroom, brush teeth, clean dentures, etc.)
  • Check rooms for foodarticles (i.e. food in proper container, unauthorized food items, etc.)
  • Maintain intake/outputrecords as assigned.
Safetyand Sanitation

·Applyrestraining device to residents in chair/bed as ordered.

·Checkresidents with restraining device at least every thirty (30) minutes or asordered.

·Releaserestraining device at least every two (2) hours for range of motion exercises,taking to bathroom, etc.

·Maintaina record of residents with a restraining device, the time devices werereleased, and how long the restraining device was released.

·Participatein appropriate in-service training programs prior to performing tasks thatinvolve potential exposure to blood/body fluids.

·Washhands before and after performing any service for the resident.

·Keepthe nurses’ call system within easy reach of the resident.

·Immediatelynotify the RN Team Leader of any resident leaving/missing from the facility.

·Followestablished safety precautions in the performance of all duties.

·Keepresidents’ personal possessions off the floor and properly stored.

·Keepfloors dry. Report spills immediately.

·Keepexcess supplies and equipment off the floor. Store in designated areas.

·Clean,disinfect, and return all resident care equipment to its designated storagearea after each use.

·Reportall hazardous conditions and equipment to the RN Team Leader immediately.

·Reportall safety violations.

·Followestablished smoking regulations. Report all violations.

·Reportany communicable or infectious disease to the DONand/or the Infection Control Coordinator.

·Followestablished isolation precautions and procedures.

·Washhands before entering and after leaving an isolation room/area.

·Followestablished procedures in the use and disposal of personal protectiveequipment.

·Wearand/or use safety equipment and supplies (i.e. back brace, mechanical lifts,etc.) when lifting or moving residents.

·Reportmissing/illegible labels and MSDSs to your supervisor.

Equipmentand Supply Functions

·Useonly the equipment you have been trained to use.

·Operateall equipment in a safe manner.

·Reportdefective equipment to the RN Team Leader.

·Informthe RN Team Leader of your equipment and supply needs.

CarePlan Functions

·ReviewCNA care cards daily to determine if changes in the resident’s daily careroutine have been made on the care plan.

·Informthe RN Team Leader of any changes in the resident’s condition so thatappropriate information can be entered on the resident’s care plan.

·Ensurethat your notes/documentation reflect that the care plan is being followed.

ResidentRights Responsibilities

·Maintainthe confidentially of all resident care information.

·Ensurethat you treat all residents fairly, and with kindness, dignity, and respect.

·Ensurethat all nursing care is provided in privacy.

·Knockbefore entering the resident’s room.

·Reportall complaints made by the resident and/or family member to the RN Team Leader.

·Reportall allegations of resident abuse and/or misappropriation of resident propertyimmediately.

·Honorthe resident’s refusal of treatment request. Report such requests to the RNTeam Leader.

WorkingConditions

·Worksthroughout the nursing service areas.

·Movesintermittently during work hours.

·Issubject to frequent interruptions.

·Isinvolved with residents, personnel, visitors, government agency personnel, etc.under all conditions and circumstances.

·Worksbeyond normal working hours, on weekends and holidays, and in other positions,temporarily, when necessary.

·Issubject to callback during emergency conditions.

·Attendsand participates in continuing education programs.

·Issubject to injury from falls, burns from equipment, odors, etc., throughout theworkday, as well as to reactions from dust, plastics, synthetics,disinfectants, tobacco smoke, and other potential contaminants.

·Issubject to exposure to infectious waste, disease, conditions, etc. including TBand the AIDS and Hepatitis B viruses.

·Maybe subject to the handling of and exposure to hazardous chemicals.

StaffDevelopment Functions

  • Attend and participate inscheduled training and educational classes to improve nursing skills andmaintain current certification as a Nursing Assistant.
  • Attend and participate inscheduled orientation programs and activities.
  • Attend and participate inannual OSHA and CDC in-service training programs for hazard communication,TB management, and blood borne pathogens standards.
  • Attend and participate inadvance directive in-service training programs as scheduled.

JOBREQUIREMENTS:

Education
  • Mustpossess a high school diploma or GED.
  • Mustbe a Certified Nursing Assistant in accordance with the laws of this state.
  • Possesssome computer skills and understanding of documentation software programpreferred.

  • Must meet the generalhealth requirements set forth by the policies of this facility, which mayinclude a medical and physical examination.
  • Must be able to relate toand work with the ill, disabled, elderly, emotionally upset, and at times,hostile people within the facility.
  • Must be able to push,pull, move, and/or lift a minimum of 10poundsto a minimum height of 5 feet and be able to push, pull, move, and/orcarry such weight a minimum distance of 5 feet.
  • May be necessary to assistin the evacuation of residents during emergency situations.
  • Sitting: 1-2 hour per 8-hour shift. Alternates frequently to standing andwalking.
