Shifts Available: 7am-3pm, 3pm-11pm, or 11pm-7am
JOB PURPOSE:
Provides assigned patients with routine daily nursing care and services according to their assessment and care plan, as directed by the nurse supervisor.
KEY RESPONSIBILITIES:
- Applies and releases restraints, and provides exercise as needed.
- Assists patients in and out of bed and into wheelchairs using the correct transfer belt and proper body mechanics.
- Assists patients with bathing, including bed baths, tub baths, and showers.
- Assists patients with dressing, grooming, oral/denture care, hair brushing, nail trimming, skin care, and shaving.
- Helps patients participate in activities as requested.
- Assists patients to dining areas, helps with feeding as needed, and documents nourishment intake. Able to identify the four basic food groups in meals.
- Supports admissions, discharges, and transfers; understands and utilizes care plans.
- Collects stool and urine samples as per care plan; cleans bedpans, empties catheter bags, and provides catheter care.
- Uses lift equipment properly and safely.
- Offers fluids at appropriate times, including ice and water rounds; distributes water and collects used pitchers.
- Documents patient information accurately and reports on patient status and changes.
- Provides bowel and bladder training, colostomy care, and range of motion exercises.
- Provides routine care for bedfast patients, including turning patients to prevent pressure sores.
- Recognizes and reports signs of abuse or changes in condition.
- Changes bedding and linens as scheduled or needed.
- Measures and records vital signs such as temperature, blood pressure, and respiration; reports any changes.

Qualifications
Minimum Education: High school diploma or equivalent.
Licensure/Certification: Active, current, and unrestricted Certified Nursing Assistant certification in the appropriate state, in accordance with state laws.
