MUST HAVE CURRENT CNA / GNA AND/OR CMT CERTIFICATES!

The CNA / GNA / CMT is responsible for delivering direct care to clients in an in-home setting.

Starting at $15.00 per hour, based on experience.

Essential Functions :

1. Follow POC (Plan of Care) as completed and maintained by RN.
2. Assist clients with ADL/IDL in an in-home setting, including feeding, bathing, toileting, dressing/grooming, nail care, and cleaning of occupied areas.
3. Complete documentation on care provided during each visit, including vital signs, temperature, medication/food intake, and bowel movements.
4. Report any changes in the patient's condition to the RN and Care Coordinator.
5. Properly lift, move, transfer, and position clients using appropriate equipment.
6. Accompany residents to appointments and advocate for them with healthcare providers.
7. Interact with clients, families, and visitors in a warm, caring, and courteous manner.
8. Travel within 30 minutes of the assigned home location.

Position Requirements

Minimum Requirements :

• High School Diploma or GED.
• Active and unencumbered CNA / GNA / CMT license in MD / Compact States.
• Maintain current CPR/BLS certification.
• Physical ability to stoop, turn, bend, squat, kneel, lift up to 50 pounds, and stand for extended periods. Must have correctable vision and hearing, and the ability to discern colors as necessary.
