To include tub, shower, whirlpool tub, bed bath, skin care, oral hygiene, shampoo, shaving, and any other hygiene needs. Observes for skin breakdown when assisting with hygiene needs. Reports breakdown or pressure areas to a licensed nurse.•Provide clean linens as appropriate•Assists with toileting patients•Assists with positioning of patients•Assists with meal trays•Assists with monitoring vital signs as instructed by licensed staff•Monitors intake and output in collaboration with licensed staff.•Assists patient with activities as instructed by licensed nursing staff•Is familiar and competent in use of full-body lift, standing lift, ceiling lift, HoverMatt, transfer belts, wheelchairs, and use of walkers, canes, and crutches•Assists licensed nurses with other patient cares as requested and able•Assists with collecting specimens and providing treatments as appropriate•Provides a safe and comfortable environment for the patient•Is aware of emergency procedures and role in each situation•Answers patient call lights promptly•Utilizes hand-off tool at start of shift; review with Licensed Nurse•Reports problems, changes, or questions to the Licensed Nurse•Attends care conferences as requested.
