Job summary

Whole Person Care Clinic seeks a compassionate Certified Nursing Assistant to support clients in recuperative care. You will assist with ADLs, basic health monitoring, and provide trauma-informed, culturally responsive care.

Collaborate with LVNs, MAs, case managers, and the care team to promote safety, recovery, and well-being. Required CNA certification and completion of approved training; BLS preferred or within two weeks of hire.

Qualifications

  • Completion of a state‑approved CNA training program.
  • Current CNA certification in applicable state is required.
  • BLS certification preferred or within 2 weeks of hire.

Responsibilities

  • Assist with bathing, grooming, dressing, toileting, and hygiene.
  • Assist with eating, drinking, and meal setup.
  • Assist with transfers, ambulation, and mobility per safety procedures.
  • Maintain client dignity, privacy, and confidentiality.
  • Monitor vital signs and report changes per CNA scope.
  • Document timely in the EHR and communicate updates to the care team.
  • Provide emotional support and engage clients in activities.
  • Follow infection control procedures and PPE usage.

JOB SUMMARY

The Certified Nursing Assistant (CNA) supports clients recovering from illness, injury, or medical treatment within WPCC's Recuperative Care facility. The CNA provides compassionate, person-centered assistance with activities of daily living, personal hygiene, mobility, comfort, and basic health monitoring.

The CNA works with the Medical Coordinator (LVN), Medical Coordinator Assistants (MAs), case managers, and other care team members to support client safety, recovery, independence, and well‑being, using a trauma‑informed, respectful, and culturally responsive approach.

KEY RESPONSIBILITIES
Personal Care
  • Assist with bathing, grooming, dressing, toileting, and hygiene
  • Assist with eating, drinking, and meal setup
  • Assist with transfers, ambulation, positioning, and mobility per safety procedures
  • Help with bed changes, laundry, and organizing client living areas
  • Encourage client independence and participation in their own care
  • Maintain client dignity, privacy, and confidentiality
Health Monitoring and Clinical Support
  • Monitor and report changes in client condition, behavior, appetite, and well‑being
  • Obtain and document vital signs within CNA scope of practice
  • Observe for symptoms requiring further assessment and report promptly
  • Follow infection control procedures and use PPE
  • Help maintain a safe environment for clients, staff, and visitors
Client Support and Engagement
  • Provide emotional support, encouragement, and companionship
  • Maintain respectful, professional relationships and boundaries
  • Encourage participation in social and recreational activities
  • Communicate effectively across language, cognitive, or behavioral barriers
  • Provide trauma‑informed, culturally responsive care
Documentation and Communication
  • Maintain accurate, timely documentation in the EHR
  • Complete shift documentation and communicate updates to the care team
  • Report changes in condition or behavior per procedure
  • Complete incident reports and notify supervisors/clinical staff as needed
  • Maintain confidentiality per organizational and privacy policies
Care Coordination
  • Collaborate with the care team to support continuity of care
  • Reinforce care instructions from licensed clinical staff
  • Help clients prepare for appointments and daily routines
  • Report barriers to care or changes in client needs
Environmental Safety and Infection Control
  • Maintain clean, safe, sanitary client and common areas
  • Maintain linens and hygiene/care supplies
  • Properly handle and dispose of biohazardous materials and sharps
  • Report maintenance or safety concerns to site management
  • Stock and replenish client care supplies
REQUIREMENTS & QUALIFICATIONS
Education
  • High school diploma or equivalent preferred
  • Completion of a state‑approved CNA training program
Experience
  • Previous CNA experience preferred
  • Experience in skilled nursing, recuperative care, assisted living, hospitals, or shelters preferred
  • Experience with complex medical, behavioral health, or social needs a plus
Certifications
  • Current CNA certification in applicable state required
  • Current BLS certification required (or within 2 weeks of hire)
  • Additional healthcare certifications preferred, not required
