Role Summary

Our client is seeking a compassionate and dedicated Personal Care Specialist to join their interdisciplinary team and make a positive impact on the lives of patients. This role involves assisting with Activities of Daily Living (ADLs), providing personal care, and contributing to the wellbeing of patients in a supportive and collaborative environment. As a member of the care team, you will have the opportunity to foster relationships, support individualized care plans, and create a safe, nurturing environment for patients.

Duties
  • Deliver high-quality care as part of an interdisciplinary team, ensuring the implementation of personalized care plans for patients across multiple settings, including the center, clinic, contracted facilities, and patients' homes.
  • Provide personal care assistance, including bathing, dressing, grooming, feeding, toileting, and other essential daily living activities.
  • Facilitate and participate in group and individual activities aligned with the daily activity calendar to engage and support patients.
  • Assist with meal preparation, service, monitoring, and cleanup, ensuring that all dietary needs are met.
  • Maintain a clean, safe, and organized environment for patients throughout the day.
  • Record and monitor vital signs, weights, and changes in the physical or emotional health of patients, reporting findings to the appropriate team members.
  • Accompany patients to appointments and assist with transportation, ensuring patient safety and comfort.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans, addressing patient needs and changes in condition.
  • Maintain accurate and up-to-date documentation in patient medical records, ensuring compliance with charting and reporting requirements.
  • Uphold patient confidentiality and protect the privacy of all company procedures, results, and sensitive information.
  • Foster positive, caring relationships with patients, families, colleagues, and community members.
  • Participate in team meetings, in-service training, and ongoing education to support quality improvement efforts.
  • Adhere to organizational policies, procedures, and OSHA safety guidelines while maintaining professionalism and confidentiality.
  • Participate in continuing education classes and required staff training meetings.
  • Perform additional duties as assigned to ensure the well‑being and care of patients.
  • Other duties as assigned.
Required Qualifications
  • Two years of experience working in a hospital, nursing home, or community‑based setting is preferred.
  • CNA license.
  • At least one year of experience working with frail or elderly populations preferred. Training will be provided upon hiring if this experience is not present.
  • Position‑specific competencies for the Personal Care Specialist must be met before assuming patient care duties.
Desired Qualifications

  • Knowledge of the principles, practices, and standards related to CNA work.
  • Experience with EMR and proficiency in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
