Description
- Attends Interdisciplinary Group Meetings (IDG) as requested.
- Under the direction and ongoing supervision of the registered nurse (RN), provides physical care of patient and their surroundings which may include bathing patient, oral, skin and hair care. The CNA performs homemaker services which may include, changing linens on patient bed, preparing meals, feeding patient, and other housekeeping duties for patient as directed in Plan of Care.
- Assists patient with ambulation and transfer, assists patient with range-of-motion exercises/positioning, reinforces dressings as needed; follows infection control procedures and Standard Precautions against Bloodborne Pathogens and other duties as assigned by RN Case Manager or RN Team Lead according to the CNA task list.
- Provides emotional support and compassionate care to the patient and family unit. Demonstrates listening skills, offers encouragement when appropriate, and practices patience in voice and manner.
- Assists other Hospice team members in the instruction of the primary caregiver and family, re: physical care, disease process.
- Attends/completes 12 hours of in-services training each calendar year.
- Offers support and assistance to the primary caregiver and family during the disease process and hospice program involvement.
- Reports any changes in the patient’s medical, nursing, rehabilitative and social needs to the Case Manager or hospice office to notify the RN.
- May be assigned to inventory patient supplies and equipment; order and track patient supplies and equipment and ensure the supply room is clean and organized. Does laundry. Uses supplies/equipment judiciously.
- Moves patients in a manner consistent with safety practices which includes use of proper ergonomic lifting/moving techniques and training, in order to maintain a safe environment.
- Reports to work and required meetings on time and in a consistent, regular, and reliable manner.
- Participates in Quality Improvement activities.
- Adheres to organizational policies and procedures.
- Follows infection control policies and procedures; utilizing PPE as required.
- Follows building protocol.
- Attends mandatory education in-services and meetings.
- Maintains confidentiality according to HIPAA requirements of patient and family regarding disease/diagnosis, financial, and other information obtained during employment.
- Abides by agency proprietary confidential information policy.
- Demonstrates the Model visit on each visit encounter.
- Follows safety education including physical ergonomics.
- Maintains personal and professional boundaries.
- Provides accurate and timely documentation, including pertinent and complete data in the electronic medical record, according to documentation guidelines.

Job Summary: The Certified Nursing Assistant provides physical and personal care to the hospice patient under the direction of the registered nurse. To provide assistance and understanding to the primary caregiver and family unit in a professional, courteous, supportive manner. Promotes professional integrity, exceptional respect, teamwork and participates in the interdisciplinary team, agency functions, meetings, and training, as required. Provides continuous care coverage as needed. To work as a hospice team member to provide care within the guidelines of the interdisciplinary group (IDG) care plan.

Northern Illinois Hospice supports our employees’ wellbeing and regularly benchmarks with other companies in our industry to offer an extensive Total Reward package for this position. The expected base salary range for this role is $18.50 - 23.87 per hour
. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.

Requirements
Education: High school diploma or general education degree (GED)
Training and Licensure: Illinois State Nursing Assistant Certificate
Experience: One year or more nursing home or home care experience in a supervised setting preferred.
Knowledge/Skills/Ability
- Must meet state or federal CNA training and competency requirements.
- Ability to work independently, make accurate, and at times, quick judgements.
- Ability to respond appropriately to crisis in home setting.
- Acceptance of and adaptability to different social, racial, cultural, and religious modes.
