Job summary

Sapphire Center for Rehab and Nursing is a 5 Start long-term skilled care facility in New York seeking enthusiastic CNAs for all shifts. Under supervision, CNAs provide direct nursing care and assist residents with personal needs to maintain dignity and comfort.

Responsibilities include hygiene, feeding, transfers, vital signs, documentation, and ensuring a clean, safe environment. New graduates are welcome and training is provided; prior long-term care experience is a plus.

Under supervised professional and skilled guidance, our CNAs give direct nursing care to assigned residents and perform other services necessary for their personal and comforts needs. Job Responsibilities / Duties:

• Promotes resident hygiene and comfort
• Distributes food/water and feeds residents
• Assists with repositioning and transferring residents
• Ambulates and transports residents
• Records and totals fluid intake and output
• Weighs residents
• Documents activities on flow sheets
• Assists with admission, discharge and postmortem care
• Responds to call lights in a timely manner
• Performs perineal care and uncomplicated colostomy care
• Reports all observations & assessments to the Team Leader/Nurse Manager/Charge Nurse
• Takes and records vital signs
• Makes beds and keeps resident’s units clean and in order
• Prepares room for housekeeping after resident discharge

Skill / Experience Requirements.

• Current NY CNA Certification.
• Previous Long Term Care experience a plus.
• New Graduates Welcome
• Willing to Train
