Job Responsibilities

• Providing nursing care to post-acute residents in nearby areas
• Monitoring residents’ conditions and updating the nursing team
• Measuring and documenting vital signs
• Assisting residents with grooming, feeding, toileting, and ADLs
• Maintaining hygiene and safety standards
• Transporting residents and assisting with mobility
• Engaging with residents to provide comfort and build trust
• Following care plans and adhering to regulations and policies
• Communicating effectively with staff, residents, and families
