Job summary

Passion Hospice PRN CNA provides direct patient care under the supervision of the RN Case Manager, following the Aide Plan of Care and CMS/CHAP standards to ensure patient comfort, safety and dignity.

The role requires travel across Northern San Antonio and Boerne; bilingual Spanish is preferred and EMR experience (Kinnser/WellSky) is a plus. This PRN position offers flexibility with potential for expanded hours as census grows.

Qualifications

• High School Diploma or GED preferred.
• Completion of CMS §418.76 competency pathways within last 24 months.
• Current listing in good standing on the Texas Nurse Aide Registry.
• Hospice or elderly-care experience preferred.
• EMR experience with Kinnser or WellSky preferred.
• Valid driver’s license, auto insurance, and reliable transportation.
• Ability to speak English; Spanish bilingual preferred.

Responsibilities

• Direct Patient Care including hygiene, grooming, toileting, and mobility assistance.
• Plan of Care Implementation with required visit frequency.
• Observation and Reporting of patient condition changes to RN.
• Infection Control and maintaining a clean, safe environment.
• Documentation in the EMR accurately and timely.
• Communication with patients, families, and staff.
• Safe Transfers and Ambulation; Operational Support and time management.

Position Summary

The PRN CNA provides direct patient care under the supervision of the RN Case Manager and in accordance with the Aide Plan of Care. This role supports patient comfort, safety, and dignity while ensuring compliance with CMS Conditions of Participation and CHAP standards.

Minimum Qualifications

• High School Diploma or GED preferred


• Completion of one of the CMS §418.76 competency pathways within the last 24 months


• Current listing in good standing on the Texas Nurse Aide Registry


• Hospice or elderly‑care experience preferred


• EMR experience preferred (Kinnser/WellSky)


• Valid driver’s license, auto insurance, and reliable transportation


• Ability to speak, read, and write in English; Spanish bilingual preferred

Key Responsibilities
