Location: Montgomery County

Schedule: Monday-Friday 6a-2p or 7a-3p

Primary Job Responsibilities

1. Providing frail, ill, or immobile clients with assistance, company, and comfort
2. Executing the patient care plan and communicating effectively with the RN Case Manager
3. Monitoring clients' physical, mental, and emotional conditions and reporting changes
4. Performing personal hygiene care
5. Performing light housekeeping in the patient's living area
6. Following nutritional guidelines for patients
7. Preparing meals and assisting with eating and drinking
8. Assisting with incontinence care and bathroom visits
9. Running errands, including shopping for groceries and household items for the patient
10. Assisting clients with mobility, such as transferring from beds to chairs or wheelchairs
11. With prior approval, accompanying clients on trips like medical appointments
12. Maintaining a clean, safe environment
13. Documenting care using the Agency-approved Electronic Visit Verification system
14. Communicating scheduling needs and changes promptly
15. Administering medicines and treatments with proper training and authorization

Requirements

1. Effective reading, writing, and communication skills
2. Ability to understand written and verbal instructions
3. Valid Certified Nursing Assistant License in Pennsylvania
4. Successful completion of a knowledge exam specific to the role
5. Strong organizational skills
6. Compassionate attitude towards patient care
7. Ability to accurately document actions and observations
8. Preferred: One year of home health experience
9. Proof of COVID-19 and Influenza vaccinations or approved exemptions
10. Driver's license and auto insurance if in a driver role
