Job summary

Altea Healthcare is seeking a Certified NP to serve as a key clinical provider in skilled nursing facilities across Brookville and Liberty, IN. The role requires diagnosing, treating, and guiding patients with a patient-first approach while traveling between sites.

Ideal candidates have geriatrics, internal medicine, or primary care experience, strong communication, and be eligible for an NP license. Training and mentorship are provided for all experience levels.

Qualifications

• Certified NP with license or eligibility to obtain
• Experience in geriatrics, internal medicine, or primary care preferred
• Strong communication and teamwork skills
• Willingness to travel to multiple facilities in Brookville and Liberty, IN

Responsibilities

• Perform clinical assessments, exams, and histories
• Diagnose and manage acute and chronic conditions
• Order and interpret labs and diagnostic tests
• Prescribe medications and develop treatment plans
• Collaborate with physicians, nurses, and facility staff
• Educate patients and families on conditions and preventive care
• Maintain accurate documentation of patient care

What You’ll Do:

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

Your daily responsibilities include:

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
