Altea Healthcare is seeking a Certified Nurse Practitioner for post-acute and skilled nursing care at facilities in the Lakeport area with travel to Salinas, CA. The role offers a salary up to $165,000 per year plus uncapped bonus potential and full benefits.

The NP will diagnose, treat, and manage patient care in skilled nursing facilities, collaborating with physicians and staff, while providing education to patients and families.

Qualifications

• Licensed NP with current certification as Nurse Practitioner.
• Experience in post-acute or skilled nursing care preferred.
• Willingness to travel between facilities (Lakeport and Salinas areas).

Responsibilities

• Perform physical exams and review patient histories.
• Order and interpret diagnostic tests (labs, imaging).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and develop treatment plans.
• Collaborate with physicians, nurses and facility staff.
• Educate patients and families on conditions and preventive care.
• Document care accurately and efficiently.

What You’ll Do:

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

Your daily responsibilities include:

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
