Job summary

CommuniCare+OLE is seeking a part-time Certified Nurse Midwife in Fairfield, CA to provide ambulatory and outpatient care. You will deliver prenatal and postpartum services, coordinate with CPSP and medical providers, and support health promotion initiatives in a diverse community.

Responsibilities

• Provides and manages direct patient care, including examinations, evaluations, diagnoses and treatment.

• Refers patients to CPSP and Sweet Success services.
• Collaborates with diabetic educator in managing Care Plans for diabetes in pregnancy.
• Furnishes pharmaceuticals and treatment regimens within scope of practice.
• Provides sexual and reproductive health services and preventive care.
• Participates in Prenatal Case Conference with Providence Medical’s Obstetrics and Gynecology group.
• Helps develop Clinical Protocols and Guidelines for Prenatal/Postpartum care.
• Directs and coordinates nursing and support staff activities as required.
• Follows departmental policies and participates in quality and safety initiatives.

JOB SUMMARY/OVERVIEW:

The Certified Nurse Midwife (CNM) is responsible for providing and managing ambulatory and outpatient quality health care services to patients of CommuniCare OLE. The CNM directly reports to the Director of PN Services. The CNM provides prenatal care, practices strong communication between prenatal providers, CPSP providers, and community providers.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

• Master of Science in Nursing, CNM certification
• State of California CNM License

• Experience/Lived Experience:

Experience In a Community Healthcare Center Preferred

• Special Skills/Training:
• Bilingual in English/Spanish preferred
• Must certify and remain current in CPR certification

Essential Functions And Responsibilities

Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population. Consult with staff physicians and Family Medicine providers as needed. Refers patients to specialists and to relevant patient care resources as appropriate.

Completes documentation of medical services within the EMR and attends to administrative patient care tasks in a timely fashion and in accordance with CommuniCare + OLE policies and standards.

Provides prenatal and postpartum care for low - moderate risk pregnancies. Consultation with Obstetrician for co-management of specified medical complications encountered during pregnancy.

Refers all eligible patients to CPSP and Sweet Success services.

Collaborates with diabetic educator in managing Care Plans for diabetes in pregnancy.

Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within their scope of practice.

Provides sexual and reproductive health services, including preventive care, to ensure highest level quality of care and access to care at CommuniCare+OLE.

Collaborates with the Quality Improvement (QI) team to develop, implement and promote the QI initiatives related to sexual and reproductive health, including prenatal care and cancer screening.

Participates in Prenatal Case Conference with Providence Medical’s Obstetrics and Gynecology group.

Helps develop and implement Clinical Protocols and Guidelines for Prenatal/Postpartum care at CommuniCare+OLE.

Participates in recruitment and orientation of prenatal care providers, medical providers and nursing staff.

Directs and coordinates the patient care activities of nursing and support staff as require.

Follows established departmental policies, procedures, and objectives. Continuous quality improvement objectives, and safety, environmental, and/or infection control standard.

As appropriate to the position, participates in specific health promotion, education and/or prevention programs.

Performs other duties as assigned.
