Position Summary

Provides comprehensive care of adolescent and adult women in reproductive health, focusing on pregnancy, childbirth, the postpartum period, and the family planning and gynecological needs of women. The certified nurse-midwife practices within a health care system that provides for consultation, collaborative management or referral as indicated by the health status of the client. In conjunction with the health center’s Chief of OB/GYN, midwives, obstetricians and Chief Medical Officer, develops clinical protocols for nurse-midwifery practice. This key team member works as part of a holistic primary care clinic – working collaboratively with primary care providers, behavioral health clinicians, community health workers, nursing staff, addictions specialists, pharmacists, eye care providers, dentists, infectious disease providers and other clinical specialists in the service of equitable, accessible, comprehensive, patient centered care.

Principal Duties and Responsibilities
  • Provides direct patient care services within the area of expertise. Assesses, plans, documents and evaluates patient needs and treatments. Ensures proper follow up.
  • Care provided by nurse-midwife incorporates all essential factors of primary care and case management that include evaluation, assessment, treatment, and referral as required. The model of health care practiced by nurse-midwife focuses on the ambulatory care of women and emphasizes health promotion, education, and disease prevention.
  • Conducts comprehensive outpatient care including prenatal and postpartum care, family planning services, routine gynecological exams, and gynecological care of women with minor acute problems within the confines of the health center’s objective and policies in collaboration with gynecologist/obstetrician.
  • Has practice privileges and attends deliveries at a designated local community hospital. All certified nurse-midwives have a current, signed clinical practice agreement for medical consultation, collaborative management, or referral with an active Medical Staff member of BNHC who has full obstetrical privileges at the delivery hospital.
  • Must be available for attending deliveries, in-patient admissions, c-section assists and telephone triage based on hospital coverage schedule.
  • Organizes and conducts routine chart review and case management discussions with collaborating physicians of all obstetrical clients.
  • Incorporates case management activities into practice and collaborates with multidisciplinary health care team to provide continuity of care for patients.
  • Maintains collaborative relationships with other providers, administration, and other clinical staff. Furnishes professional guidance and assistance to colleagues and other staff. Utilizes skills and expertise of other disciplines to ensure comprehensive care for all patients.
  • Manages the care of women with normal pregnancies during preconception, prenatal, childbearing, and postpartum period. Manages diagnostic and therapeutic regimens for minor acute and chronic gynecological illness and seeks consultation when appropriate. Co‑manages high‑risk pregnancies and pregnancies with complications with consultant obstetrician and perinatologist.
  • Initiates follow‑up and specialty referrals for those clients needing further work‑up and/or monitoring of a current situation.
  • Provides assessment, education, and management of comprehensive family planning care including contraception, natural family planning, and pregnancy testing.
  • Educates patients so that they develop an understanding of the importance of following prescribed therapeutic regimens during pregnancy and postnatal period.
  • Coordinates the referrals to early pregnancy classes, childbirth classes, breastfeeding, and parenting classes.
  • Collaborates in the development and revision of department policies, procedures, and protocols.
  • Participates in discussion with other clinical and administrative staff around the development of goals, objectives and plans for the resolution of problems in the perinatal health services.
  • Demonstrates consistent professionalism and reliability, including dependable attendance.
  • Treats all colleagues with respect (clinical and non‑clinical); and actively contributes to a positive, collaborative team culture within OB/GYN departments and across the organization.
  • Expected to meet annual clinical encounter expectations by completing a minimum of 3,800 fully documented patient encounters per year (prorated based on the number of hours scheduled to serve patients in the clinic vs. Hospital).
Qualifications
  • Current Massachusetts licensure for R.N. in expanded role.
  • Certification eligible as Certified Nurse-Midwife.
  • Federal and State licensure for prescriptive writing privileges.
  • CPR/BLS certified.
Working Conditions

Works in a busy community health center in downtown business district and local community hospital. Long hours are frequently required. Must be available for some evenings and weekends, with short notice, to represent the interests of the health center. 24‑hour hospital coverage required.

Physical Requirements

Physical demands require lifting small pieces of equipment, patients, and/or boxes not to exceed 50 pounds. Mostly standing and walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversation with and/or without assistive audio devices.
