Job summary

Heritage Park Skilled Nursing & Rehabilitation Center, a Generations Healthcare facility, in Upland, CA is seeking a Certified Nursing Assistant to provide compassionate daily patient care in accordance with the resident’s care plan.

This part-time evening shift (3pm–11pm) with weekend availability requires a CA CNA license; you will assist residents with hygiene, meals, vital signs, mobility, and documentation while upholding safety and quality care.

Qualifications

• Current CA Licensed Certified Nursing Assistant.
• Ability to communicate effectively.
• Excellent customer service and the desire to be part of a dynamic team.
• Worked in prior SNF setting.

Responsibilities

• Provide excellent patient care to residents
• Assist residents with daily routines such as personal hygiene, bath functions, taking vitals, meals, bed change, etc.
• Follow safety procedures and ensure safety of residents
• Lifting or transporting into and out of beds, wheelchairs, showers, chairs, etc.
• Report and update records on resident’s conditions
• Follow instructions from the Charge Nurse/ Supervisor

Job Description:

The primary role of the Certified Nursing Assistant is to provide quality patient care to your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your Supervisor.

Essential Duties (include but are not limited to):

• Provide excellent patient care to residents
• Assist residents with daily routines such as personal hygiene, bath functions, taking vitals, meals, bed change, etc.
• Follow safety procedures and ensure safety of residents
• Lifting or transporting into and out of beds, wheelchairs, showers, chairs, etc.
• Report and update records on resident’s conditions
• Follow instructions from the Charge Nurse/ Supervisor
