Job summary

Family Health Centers of San Diego is seeking caring and professional aides to provide personal care to older adults in home and community settings, ensuring comfort, dignity, safety, and high-quality support across IADLs and daily living tasks.

Required: California Certified Nurse Assistant certificate, current CPR/AED, and ability to drive a company vehicle. Bilingual Spanish/English preferred; we offer a competitive salary and comprehensive benefits package.

Qualifications

Job description

For more than 55 years, Family Health Centers of San Diego’s (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons. FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy. Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women’s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management. Job Roles

• Provide care in a manner that ensures participant satisfaction and retention, and is in compliance with all regulatory requirements and guidelines.
• Assist with instrumental Activities of Daily Living (IADLs) across care settings.
• Assist participants in their home with light housekeeping, medication reminders, and meal preparation.
• Provide personal care to include bathing, dressing, toileting, oral and denture care, hair and skin, shaving, nail trimming, etc.
• Help participants to move, turn, get in and out of bed, chair or wheelchair, and ambulate with or without assistive devices.
• Provide physical assistance and attendant care across care settings.
• Identify safety hazards and emergencies.
• Initiate corrective action as appropriate and report the situation to the charge person/supervisor.
• Measure and record vital signs/weights and report information to appropriate medical staff following established procedures and protocols.
• Observe and report to immediate supervisor any changes in participant’s physical or mental status; record such observations on appropriate records.
• Participate in case conferences with other members of the interdisciplinary care team as appropriate.
• Help maintain order and cleanliness of the multi-purpose PACE Center.
• Assist with dining area set-up and distribution of meals and snacks as assigned.
• Assist with multi-purpose PACE Center activities as assigned.
• Perform other duties as assigned.
• Driving is an essential job function and involves driving a company vehicle.

Requirements

• High School diploma or equivalent GED Certificate required. Or an equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties and meets any required state or federal certification requirements.
• Possess a California Certified Nurse Assistant Certificate required.
• Current American Heart Association Heartsaver CPR AED Certification required within 90 days of employment.
• Meet organizational driver requirements.
• Experience/Specialized skills (including Language) Experience working with the elderly in a skilled nursing facility, residential care facility, hospital setting, or within a home setting preferred.
• Able to work effectively with culturally, economically, and educationally diverse populations.
• Demonstrated positive interpersonal skills in dealing with a wide range of staff and/or patients’ essential forms of living.
• Able to perform a wide range of duties in caring for the older adult population.
• Able to assist with personal care in homes and/or at health-related facilities to include wheelchair and ambulatory transfer between home and various transportation modes.
• Able to handle difficult situations in a professional manner.
• Strong interpersonal skills with the ability to get along well with others and gain cooperation.
• Bilingual Spanish/English preferred.
