Must be a Certified Nurse Assistant in the state of Connecticut

Certified Nurse Assistant Position Purpose:

Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.

What You Will Do:

  1. Follow care plan developed by RN.
  2. Report client changes in condition to RN, as described on care plan.
  3. Assist clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
  4. Assist clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making beds, and laundering clothes and bedding.
  5. Measure and record client’s oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered.
  6. Perform range of motion and other simple procedures as an extension of therapy services as assigned.
  7. Assist clients in self-administration of medication, as assigned by an RN and documented in the clinical record. Note: Home Health/Hospice Aide shall not administer medication.

Minimum Qualifications:

  1. High School diploma or GED required.
  2. Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
  3. Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
  4. Meets agency hiring criteria as defined in agency minimum standards (e.g., professional references).
  5. Ability to function with minimal supervision, maintaining professional relationships with clients and their families.
  6. Commitment to maintaining skills and adhering to agency policies and procedures.
  7. Ability to demonstrate organizational core competencies and adhere to the Code of Ethics and Compliance Program.
  8. Current Driver’s license and reliable transportation.
