Day Shift

Department: Hospice Home Health Aides - LVHSO

Day Shift, Monday - Friday; 8:00 am - 4:30 pm with weekend and holiday rotation

Summary
Provides hands-on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist. Provides other support services as a member of the care team.

Job Duties

  • Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
  • Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
  • Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate.

Minimum Qualifications

  • High School Diploma/GED
  • 1 year Experience in a healthcare setting as a nursing assistant or nursing aide.
  • Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team.
  • Knowledge of medical terminology.
  • Ability to drive to patients' homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs.
  • Ability to work in patient homes.
  • Ability to work with and support terminally ill patients and their family members/caregivers.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • CNA - Certified Nursing Assistant - State of Pennsylvania Upon Hire

Physical Demands
Lift over 50 lbs. Push/pull up to 140 lbs. Carry 80 lbs. short distances. Frequent standing/walking, squatting, kneeling, crawling, climbing, and reaching overhead.
