Responsibilities

• Provide personal hygiene care to hospice patients per Plan of Care.
• Deliver routine medical supplies to patients as needed.
• Observe and report changes in patient condition to the Nurse/Care Manager.
• Provide excellent customer service aligned with CHOT standards.
• Complete 12 hours of in-service training annually.
• May assist with patient transfers as needed.
• Provide compassionate companionship during services.
• Assignments may be in the field or inpatient units depending on census.

POSITION SUMMARY

Provides personal hygiene care to hospice patients following the patient's Plan of Care.

ESSENTIAL FUNCTIONS

• Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing.
• Deliver routine medical supplies as needed to patients.
• Observe and report any changes in the patient's condition to the Nurse/Care Manager.
• Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT, and provide excellent customer service.
• Successfully complete 12 hours of in-service training annually.
• May assist the patient with transfers as needed.
• Provide compassionate companionship to patients while delivering services.
• Depending on the census, assignments may include patients in the field or in inpatient units.
