Compensation: $17.00 - $20.00 / hour

Description

Schedule: 40 hours/week | 8:00am-4:30pm | Monday-Friday

Department: Huron Valley PACE | Monroe, MI

What You'll Do in This Role: Enjoy a career that supports both your professional passion and your personal life. At Huron Valley PACE, you’ll work a Monday–Friday schedule from 8:00 AM to 4:30 PM with no weekends, holidays, on-call, or mandating! Our team members also enjoy flexibility throughout the workday, with opportunities to work between clinic and home care settings. You’ll be part of a supportive, collaborative team that works together to provide exceptional care while creating a positive and balanced work environment.

This role blends hands-on clinical care with administrative support to keep our clinic running smoothly. You’ll assist with exams, procedures, documentation, and participant coordination — all while supporting a mission that helps older adults age with independence. Because this is a community-based PACE program, you’ll also play an essential role in supporting care both onsite and out in the community. Candidates with home care or community-based field experience are strongly encouraged to apply, as these skills help ensure we deliver seamless, person-centered care across settings.

Key Responsibilities

Clinical Support & Patient Care

  • Partner with nurses, physicians, and other team members to ensure smooth clinic flow and participant care.
  • Welcome participants, escort them to exam rooms, and assist with comfort and positioning during visits.
  • Take and accurately record vital signs, including height, weight, oxygen saturation, and blood pressure.
  • Assist with lab work, specimen collection, and routine ADL (Activities of Daily Living) care.
  • Document care, observations, and procedures in participant records.
  • Communicate changes in participant condition promptly to nursing and interdisciplinary team members.

Clinic Operations & Administrative Support

  • Keep the clinic organized and running efficiently by managing the daily schedule and coordinating appointments.
  • Communicate with participants, families, and providers to ensure seamless care coordination.
  • Confirm and reschedule appointments as needed while maintaining a friendly, professional approach.
  • Prepare exam rooms, manage clinic supplies, and ensure equipment is clean and ready for use.
  • Support administrative tasks such as answering phones, faxing, scanning, and maintaining documentation.

Data & Quality Support

  • Collect and maintain accurate participant data and assist with quality improvement (QI) initiatives.
  • Support ongoing documentation needs and help track key clinical and operational metrics.

Team Collaboration

  • Participate in daily huddles and interdisciplinary team meetings.
  • Contribute ideas, share insights, and help create a positive, collaborative work environment.
  • Support learning opportunities for staff and teammates.

Other Responsibilities

  • Occasionally assist in the Day Center or during participant home visits.
  • Adhere to all Huron Valley PACE policies and uphold our mission to provide exceptional, person-centered care.

The above is a summary of the position, it in no way states or implies that these are the only duties this position will be required to perform. If selected for the position you will receive a full job description.
