You’ll be part of a supportive, collaborative team that works together to provide exceptional care while creating a positive and balanced work environment.
This role blends hands-on clinical care with administrative support to keep our clinic running smoothly. You’ll assist with exams, procedures, documentation, and participant coordination — all while supporting a mission that helps older adults age with independence. Because this is a community-based PACE program, you’ll also play an essential role in supporting care both onsite and out in the community. Candidates with home care or community-based field experience are strongly encouraged to apply, as these skills help ensure we deliver seamless, person-centered care across settings.
