Job description

Responsible for providing direct patient care, ongoing education and reinforcement of care plans, and providing general support in an ambulatory setting under the supervision of the patient's physician or LIP/RN designee, in accordance with policy, procedure, and competency to promote patient health and wellness.

Position Overview

This position requires delivering services to all age groups, demonstrating an understanding of the functional and developmental age of the individuals served.

Responsibilities
  1. Assist the registered nurse and/or provider with data collection regarding the physical, psychological, social, and cultural aspects of patients, following practice standards and institutional policies.
  2. Organize and prioritize nursing care activities based on patient needs and interdisciplinary team input.
  3. Implement age-appropriate interventions as directed by the registered nurse or provider.
  4. Assist in evaluating the effectiveness of care provided, ensuring progress toward desired outcomes.
  5. Provide ongoing education and reinforcement of care plans developed by the Provider/RN, tailored to the patient's health, education needs, condition, and age.
  6. Ensure a safe environment and the safe delivery of care.
  7. Take responsibility for personal professional development and support the professional growth of colleagues.
  8. Demonstrate teamwork in patient care delivery.
  9. Perform other duties as assigned, in addition to primary responsibilities.
Position Details

Compensation Range: $18.37 - $28.47 Hourly

Minimum Requirements
  • Education: Graduation from an accredited Licensed Practical Nurse Program.
  • Experience: No prior experience required.
  • Licensure: Valid Practical Nurse (LPN) license in Virginia before onboarding. AHA BLS certification is required. A valid Virginia Driver's License may be required.
Physical Demands

The role may involve prolonged standing, frequent travel, bending, and stooping. Requires proficient communication, auditory, and visual skills, attention to detail, legible writing, and the ability to lift/push/pull 20-50 lbs. May involve exposure to chemicals, blood, body fluids, and infectious diseases.
