Job summary

Live Well Homecare is seeking a licensed Physical Therapist to plan and implement treatment programs for patients across all ages, following established practices. You will collaborate with an interdisciplinary team to deliver comprehensive care and develop individualized treatment plans that adapt to each patient’s needs.

This role emphasizes teamwork, clear communication, and evidence-based approaches in a home health setting.

Qualifications

• Requires a high school diploma or GED, a Doctorate in Physical Therapy (DPT) or equivalent, and licensure from the appropriate state board.
• Certification in Basic Life Support (BLS) and specialized certifications such as Certified Lymphedema Therapist (CLT) are preferred.
• Experience of at least one year in clinical settings is advantageous.
