Job summary

Legacy Hospice, Inc. is hiring a Certified Nursing Assistant (CNA) to provide compassionate, in-home hospice care in Gulfport, MS. You will perform personal care tasks, support families, and monitor patient conditions under the RN Case Manager's guidance.

The role requires an active CNA license in MS, current BLS/CPR, and a valid driver's license. A minimum of 1 year in hospice/home health is preferred, with pre-employment drug screening and a team-oriented, independent work approach.

Qualifications

• Active CNA certification in the state of employment.
• Minimum 1 year of nursing assistant experience (hospice/home health or community-based preferred).
• Current BLS/CPR certification.
• Valid driver’s license with reliable transportation.
• Ability to work independently and compassionately in home settings.
• Pre-employment drug screening required, including marijuana.

Responsibilities

• Provide personal care based on the individualized care plan (bathing, grooming, feeding, dressing, toileting, etc.).
• Maintain patient comfort and safety in the home environment.
• Monitor and report changes in the patient's condition to the RN Case Manager.
• Assist with transfers, mobility, and range-of-motion exercises.
• Educate and support families on patient care techniques.
• Document clinical notes accurately and on time.
• Participate in team meetings and quality improvement initiatives.

What You’ll Do

As a hospice CNA, you’ll play a vital role in helping patients and families through meaningful support, including:

• Providing personal care based on the individualized care plan (bathing, grooming, feeding, dressing, toileting, etc.)

• Monitoring and reporting changes in the patient's condition to the RN Case Manager
• Assisting with transfers, mobility, and range-of-motion exercises

• Documenting clinical notes accurately and on time

• Graduate of an approved Certified Nursing Assistant program
• Minimum 1 year of nursing assistant experience (hospice, home health, or community-based preferred)
• Current BLS/CPR certification
• Valid driver’s license, reliable transportation, and car insurance
• Ability to work independently and compassionately in home settings
• Excellent communication, professionalism, and time management skills
