Job summary

Legacy Hospice, Inc. seeks a compassionate CNA to provide in-home end-of-life care in Demopolis, AL. This part-time, hourly role supports patients and families according to individualized care plans.

You will perform personal care, monitor conditions, assist with mobility, and document findings for the RN Case Manager. Strong communication and reliability are essential.

Qualifications

• Active CNA certification in the state of employment.
• Graduate of an approved CNA program.
• Minimum 1 year of nursing assistant experience (hospice/home health preferred).
• Current BLS/CPR certification.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide personal care (bathing, grooming, feeding, dressing, toileting) per care plan.
• Maintain patient comfort and safety in the home setting.
• Monitor and report changes to the RN Case Manager.
• Assist with transfers, mobility, and ROM exercises.
• Educate and support families on care techniques.
• Document clinical notes accurately and on time.
• Participate in team meetings and quality initiatives.

We are seeking a caring and dependable Certified Nursing Assistant (CNA) to join our interdisciplinary team and provide personal, hands-on support to hospice patients in their homes.

What You'll Do

As a hospice CNA, you'll play a vital role in helping patients and families through meaningful support, including:

• Providing personal care based on the individualized care plan (bathing, grooming, feeding, dressing, toileting, etc.)
• Maintaining patient comfort and safety in their home environment
• Monitoring and reporting changes in the patient's condition to the RN Case Manager
• Assisting with transfers, mobility, and range-of-motion exercises
• Educating and supporting families on patient care techniques
• Documenting clinical notes accurately and on time
• Participating in team meetings and quality improvement initiatives

Qualifications

• Active CNA certification in the state of employment
• Graduate of an approved Certified Nursing Assistant program
• Minimum 1 year of nursing assistant experience (hospice, home health, or community-based preferred)
• Current BLS/CPR certification
• Valid driver's license, reliable transportation, and car insurance
• Ability to work independently and compassionately in home settings
• Excellent communication, professionalism, and time management skills
