Job Description

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist. They perform various personal care services to meet the patient’s needs, including observing, reporting, and documenting patient conditions and care provided.

Job Details

The home health aide is assigned by a Registered Nurse to ensure quality, continuity, and safety of patient care. Responsibilities include:

• Monitoring patient physical and mental conditions for safety
• Supporting patients with housekeeping, laundry, shopping, meal preparation, and serving
• Assisting with feeding, bathing, dressing, grooming, laundry, and self-care
• Administering medication reminders and assisting with medication administration if required
• Teaching patients to use assistive devices and techniques for personal hygiene
• Assisting with daily activities and exercises as per care plan
• Educating family members on patient care techniques
• Recording patient information and reporting changes to nursing staff
• Maintaining a safe environment following health standards and dietary requirements
• Adhering to professional, home care, and legal standards

The role may involve performing other job-related tasks as needed. The incumbent does not have supervisory responsibilities.

Minimum Qualifications

• Current California Certified Home Health Aide Certificate
• At least 1 year of supporting individuals with disabilities, mental illnesses, or challenging behaviors
• Current CPR/First Aid certification and negative TB test or chest x-ray
• Valid in-state driver’s license, registration, insurance, and a clean driving record
• Availability to work in Orange County, Santa Monica, or Burbank
